MIDI Assignments and Plugin Selector

MIDI Assignments are now working for everything that's currently supported in the UI. Still need to do audio/MIDI recorder and sequencer commands.

The plugin selector is now fully integrated with Cantabile's VST scanning and has had a few improvements since the prototype version.

• Favourite and Recent categories have been moved to the top and now save/load correctly.
• The Choose File/Plugin button now serves to open either a .dll plugin or a .fxp/.fxb file.
• There's a new button to do a quick scan on the plugin folders.

The plugin selector now supports selecting fxp and fxb files from a pre-configured presets folder, including support for sub-folders (similar to Cantabile 1.2's favourites menu).

I was going to remove this this, but I've had a few support queries about it lately so I think it's used more than I expect.
