MIDI Controller Assignments Improvements

Today I made some improvements to Cantabile's MIDI Controller Assignments - primarily the ability to create assignments to global settings for the current session.

Previously, assignments to global items (eg: master volume) could only be saved globally - not per session. There's a new setting now labelled "Context" that lets you specify if the settings should be saved for the current session or globally:

Also, cleaned up the dialog a little, disabling controls rather than hiding them and changed the formatting of some of the text in the Assign To combo.
